Following a patient visit, the care provider will send a visit summary to the patient's primary care physician and refer the patient back to the primary physician for ongoing, follow-up care—thus providing a continuity of care to enhance the patient's experience and improve outcomes. If a patient does not have a primary care physician, the CareNow staff will offer to help the patient locate one within the Centerpoint Medical Center family.
